CO2 Laser Aftercare: Essential Tips for Optimal Healing and Best Results
CO2 laser aftercare is just as important as the treatment itself — maybe even more so. I’ve seen clients with identical treatments have completely different results, and the difference almost always comes down to how well they followed their aftercare protocols.
After over 10 years of performing CO2 laser treatments in Calgary and guiding hundreds of clients through recovery, I’ve developed a comprehensive aftercare approach that maximizes results while minimizing complications. This guide shares everything you need to know to heal beautifully.
Why CO2 Laser Aftercare Matters So Much
When I perform a CO2 laser treatment, I’m creating thousands of microscopic columns of controlled thermal injury in your skin. This triggers your body’s wound-healing response — inflammation, then new tissue production, then remodeling. The quality of that healing process determines your final results.
Poor CO2 laser aftercare can lead to:
• Prolonged redness and healing time
• Increased risk of infection
• Post-inflammatory hyperpigmentation (dark spots)
• Suboptimal collagen production
• Compromised results
Proper CO2 laser aftercare supports your body’s natural healing, protects vulnerable new skin, and sets the stage for the best possible outcome. It’s not optional — it’s essential.
CO2 Laser Aftercare: The First 24 Hours
The first day is critical. Your skin is at its most vulnerable immediately after treatment, and infection risk is highest in the first 24-48 hours.
Go straight home: Don’t run errands or make stops. Your treated skin needs a clean environment immediately.
Keep skin moist: Apply the healing ointment I provide within an hour of leaving the clinic, and reapply every 2-3 hours or whenever skin feels tight. In Calgary’s dry climate, you may need to apply even more frequently than clients in humid cities.
Expect heat and tingling: A hot, prickling sensation is normal for the first 1-2 hours and gradually fades. By evening, it should feel like a mild sunburn.
Sleep elevated: Use two pillows to keep your head elevated. This minimizes swelling, especially around the eyes.
Use clean pillowcases: Change your pillowcase daily for the first 3 days. Bacteria from used pillowcases can cause infection on healing skin.
No touching: Keep your hands away from your face unless you’ve just washed them thoroughly. This is the most common way infections start.
Stay indoors: Absolutely no sun exposure. If you need fresh air, wait until after sunset.
CO2 Laser Aftercare: Days 2-4
Swelling peaks around day 2, then gradually subsides. Your skin will start developing that characteristic “sandpaper” texture as the treated tissue prepares to shed.
Continue moisture protocol: The single most important CO2 laser aftercare rule is keeping skin constantly hydrated. Never let it feel dry or tight. Apply healing ointment every 2-3 hours during the day.
Gentle cleansing: Cleanse 2-3 times daily with the gentle cleanser I recommend. Use cool or lukewarm water — never hot. Pat dry with a clean towel (don’t rub), then immediately apply healing ointment.
Manage itching: As skin heals, itching is normal and can be intense. Do NOT scratch! Use cool compresses or take an antihistamine like Reactine or Claritin if needed. Ice packs wrapped in a clean cloth can help.
Watch for oozing: Some clear or slightly yellow oozing is normal, especially at higher treatment settings. If this occurs, gently cleanse, let skin air dry for 15-20 minutes, then reapply ointment. This helps prevent the ointment from trapping excess moisture.
Stay indoors: You’re still too sensitive for any sun exposure. Continue avoiding sunlight completely.
CO2 Laser Aftercare: Days 5-7
This is when the magic happens. The dead skin layer begins to separate and shed, revealing fresh, glowing skin underneath.
Let skin shed naturally: You’ll notice the sandpaper texture starting to ball up and come off during cleansing. You can gently wipe away loose skin, but never pick, peel, or scrub. Forcing skin off prematurely can cause discoloration or extend healing time.
Continue cleansing and moisturizing: Your CO2 laser aftercare routine remains the same — gentle cleansing and consistent moisture. Don’t get impatient and skip steps.
Test sunscreen: Around day 5-7, once the dead skin has mostly shed, you can begin testing mineral sunscreen. Apply a small amount to a test area. If it stings or burns, wash it off and wait another day.
Expect pinkness: New skin will be pink. This is normal and will fade over the coming weeks as your skin continues to heal and mature.
CO2 Laser Aftercare: Weeks 2-4
Active healing is mostly complete, but your skin is still vulnerable and collagen remodeling is just beginning.
Daily sunscreen is mandatory: This is non-negotiable. New skin is extremely susceptible to UV damage and hyperpigmentation. Apply mineral SPF 30+ every single day, even if you’re mostly indoors. Calgary’s high altitude means more intense UV exposure than lower elevations.
Transition to lighter products: Once active peeling is complete, you can transition from heavy healing ointments to lighter, hydrating moisturizers. Look for products with hyaluronic acid, ceramides, or gentle hydrating ingredients.
Avoid actives: No retinol, vitamin C serums, AHAs, BHAs, or any exfoliating products for at least 4 weeks. Your new skin cannot handle these yet and you risk irritation or damage.
Mineral makeup only: If you need coverage, use mineral-based makeup once I’ve approved you to wear it (typically around day 7-10). Avoid liquid foundations or heavy products until fully healed.
CO2 Laser Aftercare: Months 1-6
The visible healing is done, but collagen production continues for months. Your CO2 laser aftercare during this period focuses on protecting your investment and supporting ongoing results.
Maintain sun protection: Continue daily SPF for life — but especially during these first 6 months when new skin is most vulnerable. Sun damage during this period can cause permanent hyperpigmentation.
Gradually reintroduce actives: Around week 6-8, you can slowly reintroduce gentle actives like vitamin C. Start with every other day and watch for irritation. Retinol can typically be reintroduced around week 8-12, starting at low concentrations.
Support collagen production: Consider supplements like vitamin C, zinc, and collagen peptides to support your body’s ongoing collagen synthesis. Good nutrition, sleep, and hydration all contribute to your final results. Learn more about how lifestyle affects results in my Best CO2 Laser Results Lifestyle Guide.
Be patient: Final results continue developing for 3-6 months. Some clients see continued improvement even at the 6-month mark as collagen remodeling completes.
What Products to Use for CO2 Laser Aftercare
I provide or recommend specific products for each phase of healing. Here’s what to look for:
Healing phase (Days 1-7):
• Gentle, fragrance-free cleanser (I provide this)
• Occlusive healing ointment — something thick that creates a protective barrier (I provide this)
• Nothing else touches your skin during this phase
Transition phase (Weeks 2-4):
• Same gentle cleanser
• Lighter hydrating moisturizer with ceramides or hyaluronic acid
• Mineral SPF 30+ (zinc oxide or titanium dioxide based)
• No actives, no exfoliants, no fragrances
Maintenance phase (Month 2+):
• Your normal gentle cleanser
• Hydrating moisturizer
• Daily SPF 30+ (essential for life)
• Gradually reintroduce vitamin C, retinol, and other actives as tolerated
What to Avoid During CO2 Laser Aftercare
Knowing what NOT to do is just as important as knowing what to do. Avoid these common mistakes:
Picking or peeling: This is the #1 aftercare mistake. Even if skin looks ready to come off, let it shed naturally during cleansing. Picking causes discoloration and can extend healing.
Sun exposure: Any UV exposure during healing can cause permanent hyperpigmentation. Stay indoors during peak sun hours for the first week, and wear SPF religiously afterward.
Hot water: Hot showers, baths, saunas, and hot tubs are off-limits. Heat increases inflammation and can damage healing skin. Use lukewarm water only.
Exercise that causes sweating: Sweat contains salt and bacteria that irritate healing skin. Avoid strenuous exercise for at least 5-7 days, longer for deeper treatments.
Active skincare ingredients: No retinol, glycolic acid, salicylic acid, vitamin C, or any exfoliating products for at least 4 weeks. These will irritate your new skin.
Makeup too soon: Don’t apply makeup until I approve it — typically day 7-10 at the earliest. When you do resume, use mineral makeup only.
Touching your face: Your hands carry bacteria. Keep them away from healing skin unless you’ve just washed them.
Alcohol consumption: Alcohol dehydrates you and depletes nutrients needed for healing. Avoid for at least 1-2 weeks after treatment, ideally longer.
CO2 Laser Aftercare for Calgary’s Dry Climate
Calgary presents unique challenges for CO2 laser aftercare that I don’t see mentioned in generic aftercare guides:
Our dry air is brutal on healing skin: Indoor humidity drops to 30-40% in winter with forced-air heating. Your skin loses moisture faster here than in humid climates. Combat this by:
• Applying healing ointment every 2-3 hours instead of 4-5
• Running a clean humidifier in your recovery room
• Drinking extra water to stay hydrated from the inside
Chinooks cause rapid changes: Our famous chinook winds can swing temperatures 20+ degrees in hours, bringing dry winds that sap moisture from skin. Stay indoors during the first few days of recovery and keep skin well-protected if you must go out.
High altitude UV is intense: At 1,000+ metres elevation, UV radiation is significantly stronger than at sea level. Even on cloudy days, UV exposure is higher here. This makes sun protection even more critical during CO2 laser aftercare.
Signs of Normal Healing vs Warning Signs
Normal during CO2 laser aftercare:
• Redness, swelling, and warmth (especially days 1-3)
• Mild oozing or pinpoint bleeding (day 1)
• Sandpaper texture developing (days 2-4)
• Itching (days 3-7)
• Skin flaking and peeling (days 5-7)
• Pink undertones lasting several weeks
• Minor breakouts from occlusive ointments
Contact me immediately if you experience:
• Severe pain or burning after the first 12 hours
• Increasing redness, warmth, or swelling after day 2-3
• Yellow or green discharge
• Fever or chills
• Cold sore outbreak
• Any signs of infection
• Anything that concerns you — when in doubt, reach out
CoolPeel vs Traditional CO2 Laser Aftercare
CoolPeel aftercare follows the same principles but is less intensive due to the milder treatment:
CoolPeel aftercare:
• 24-48 hours of redness (vs 5-10 days)
• Minimal to no peeling
• Can often return to normal activities day 2-3
• Same sun protection requirements
• Same avoidance of actives for 2-4 weeks
Traditional CO2 aftercare:
• 5-10 days of active healing
• Significant peeling phase
• Social downtime required
• Strict moisture and sun protocols
Regardless of treatment intensity, the core CO2 laser aftercare principles remain the same: keep skin moist, protect from sun, don’t pick, and be patient.
Frequently Asked Questions About CO2 Laser Aftercare
How long does CO2 laser aftercare last?
Active aftercare with healing ointments and strict protocols lasts about 7-10 days for traditional CO2 and 2-3 days for CoolPeel. However, sun protection and gentle skincare should continue for at least 4-6 weeks, and daily SPF should become a permanent habit. Collagen continues developing for 3-6 months, so supporting your skin’s health during this entire period optimizes your results.
Can I wash my face after CO2 laser treatment?
Yes — gentle cleansing is an essential part of CO2 laser aftercare. Cleanse 2-3 times daily with a gentle, fragrance-free cleanser and lukewarm water. Pat dry with a clean towel (don’t rub), and immediately apply your healing ointment. Keeping skin clean reduces infection risk while the moisture protocol supports healing.
When can I wear makeup after CO2 laser?
For traditional CO2, avoid makeup for at least 7-10 days or until I approve it. For CoolPeel, mineral makeup is usually fine by day 2-3. When you do resume makeup, use mineral-based products only for the first few weeks. Liquid foundations and heavy products can clog pores and irritate new skin.
What happens if I pick at peeling skin during CO2 laser aftercare?
Picking or prematurely removing peeling skin is the most common cause of post-laser hyperpigmentation and uneven results. When you force skin off before it’s ready, you expose raw tissue that can heal darker or with altered texture. Let skin shed naturally during gentle cleansing. I know it’s tempting, but patience protects your results.
Why is sun protection so important during CO2 laser aftercare?
New skin produced after CO2 laser treatment is highly sensitive to UV radiation. Sun exposure during healing can cause permanent hyperpigmentation — dark spots that are very difficult to treat. This risk continues for several months after treatment. Daily SPF 30+ is essential, and I recommend making it a permanent habit for long-term skin health.
Can I exercise during CO2 laser aftercare?
Avoid exercise that causes sweating for at least 5-7 days after traditional CO2 (2-3 days for CoolPeel). Sweat contains salt and bacteria that can irritate healing skin and increase infection risk. Light walking is fine after the first few days. When you return to exercise, rinse your face gently afterward and reapply moisturizer.
What should I do if my skin feels tight during CO2 laser aftercare?
Tightness means your skin needs more moisture — apply healing ointment immediately. During active healing, you should never let skin feel dry or tight. In Calgary’s dry climate, this may mean applying ointment every 1-2 hours during the day. If you’re consistently feeling tight even with frequent application, run a humidifier in your room.
When can I use retinol after CO2 laser treatment?
Wait at least 6-8 weeks before reintroducing retinol, and start with a low concentration every other night. Watch for irritation. Some clients need to wait 12 weeks depending on their skin’s sensitivity. When you do resume, use a gentle formulation and build up gradually. Retinol can actually enhance your long-term results once skin is ready for it.
